⚫ CVs, Domicile, 5 years Professional DMC / MBBS Transcript, House Job Certificate, Experience certificate (if any), Publications (intl / Pak Journal), FCPS Part-1 result, Rs. 500 Bank Draft (in the name of Commandant CMH Lahore) any bank (non-refundable), CNIC copy, 3x Passport size photographs, Police character verification. Those having foreign qualification to submit HEC/PMDC/PMC equivalency certificate.
